Our new patient forms can be downloaded and completed in the convenience of your own home.

On your first visit to our office you will be asked to fill out the required paperwork and sign a consent to treatment and a privacy agreement. You will be informed of payment options and insurance coverage.

History and Examination

After reviewing your history, a full Chiropractic exam will be performed, including SEMG and Thermal spinal scans with the results explained in detail. If necessary a referral for further imaging or a second opinion will be made.

We Explain Everything First

No treatment will be provided until you fully understand your condition and how Chiropractic can help. Once you understand, your treatment will begin using techniques that are appropriate for your situation. After your treatment you will be given home care instructions (if any) and be rescheduled.
